A RESOLUTION AUTHORIZING THE CITY MAYOR, HON. ROLEN C. PAULINO

 TO ENTER, EXECUTE AND SIGN THE  RENEWAL OF  THE MUTUAL ACCREDITATION AGREEMENT,  FOR AND ON BEHALF OF THE JAMES L. GORDON MEMORIAL HOSPITAL (JLGMH), THIS CITY WITH HEALTH PLAN PHILIPPINES, INCORPORATED, REPRESENTED BY ITS MEDICAL DIRECTOR, BENJAMIN C. SANCHEZ,JR.,MD, IN A MANNER  CONSISTENT WITH LAW

WHEREAS,  Health Plan Philippines, Incorporated is a Company engaged in the promotion and marketing of its health care programs consisting of preventive health care, in-patient and out-patient services;

WHEREAS, the James L. Gordon Memorial Hospital is a tertiary medical facility, owned by the local government of Olongapo City;

WHEREAS, pursuant to City Council Resolution No.121, Series of 2011 duly approved on December 20, 2011, Health Plan Philippines, Incorporated, requested and entered into a Mutual Accreditation Agreement with the City for additional provisions for the health care insurance of James L. Gordon Memorial Hospital patients;

WHEREAS, Health Plan Philippines, Incorporated has sought the renewal of its Mutual Accreditation Agreement, with the city, while binding itself to comply with all requirements set under local and national laws; 

NOW, THEREFORE, on motion of  City Councilor Eduardo G. Guerrero, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to authorize, as it hereby authorizes the  City Mayor, Hon. Rolen C. Paulino to enter, execute and sign the renewal of the Mutual Accreditation Agreement, for and on behalf of the James L. Gordon Memorial Hospital (JLGMH), this city with Health Plan Philippines, Incorporated, represented by its Medical Director Benjamin C. Sanchez, Jr.,MD  in a manner consistent with law.
