RESOLUTION NO. 123
(Series of 2005)

A RESOLUTION RATIFYING THE MUTUAL ACCREDITATION AGREEMENT WITH PERSONNEL WELFARE ASSISTANCE (PWA) OF THE COMMISSION ON AUDIT (COA) FOR AND IN BEHALF OF JAMES L. GORDON MEMORIAL HOSPITAL (JLGMH) ON THE
PROVISION OF HEALTH CARE AND MEDICAL SERVICES

WHEREAS, the Sangguniang Panlungsod approve Resolution No. 110, Series of 2005 entitled “A Resolution Authorizing the City Mayor James Gordon, Jr. To Enter Into A Mutual Accreditation Agreement with Personnel Welfare Assistance (PWA) Of The Commission On Audit (COA) For And In Behalf Of JLGMH On The Provision Of Health Care And Medical Services”;

WHEREAS, the body ratified the said Memorandum of Agreement (MOA) kast September 16, 2005;

WHEREAS, the MOA shall take effect on the day of its approval, thus, superseding no. 11 provision of the agreement;

NOW, THEREFORE, on motion of Councilor Marey Beth D. Marzan, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to ratify, as it hereby ratifies the Mutual Accreditation Agreement with Personnel Welfare Assistance (PWA) of the Commission on Audit (COA) for in behalf of James L. Gordon Memorial Hospital (JLGMH) on the provision of Health Care and Medical Services.
