            RESOLVED, AS IT IS HEREBY RESOLVED,, by the Sangguniang Panlungsod in session assembled, to enact the following Ordinance:

ORDINANCE NO. 02

AN ORDINANCE APPROVING AND APPROPRIATING THE AMOUNT OF

TWO HUNDRED SEVENTY EIGHT THOUSAND PESOS (PHP278,000.00) AS SUPPLEMENTAL BUDGET NO. 2014–6(LEE)  FOR CALENDAR YEAR 2014

UNDER JAMES L. GORDON MEMORIAL HOSPITAL

BE IT ORDAINED, BY THE SANGGUNIANG PANLUNGSOD IN SESSION ASSEMBED, THAT:

SECTION 1.  The Supplemental Budget No. 2014–6(LEE)  for Calendar Year 2014 under  James L. Gordon Memorial Hospital with a total amount of Two Hundred Seventy Eight Thousand Pesos (Php278,000.00)  is hereby approved.

The budget documents consisting of the following are incorporated herein and made integral part of this Ordinance:

1.      LBP Form No. 9 – Statement of Funding Sources

2.      LBP Form No. 10 – Statement of Supplemental Appropriation

SECTION 2.  Sources of Funds.  The source of funds for the Supplemental Budget in the amount of Two Hundred Seventy Eight Thousand Pesos (Php278,000.00) shall be as follows:

SECTION 4. Separability Clause.  If, for any reason, any section or provision of this Ordinance is declared invalid or unconstitutional, other sections or provisions thereof which are not affected thereby shall continue to be in full force and effect.

SECTION 5.  Effectivity.  The provisions of this Ordinance shall take effect upon its approval.
