RESOLUTION NO. 63
(Series of 2006)

A RESOLUTION AUTHORIZING THE CITY MAYOR TO ENTER INTO A MEMORANDUM OF AGREEMENT WITH MEDICAL STAFF OF OLONGAPO-CONSULTANTS’ ASSISTANCE & REHABILITATION ENDEAVOR (MSO-CARE) FOUNDATION

WHEREAS, the MSO-CARE is an association composed of consultants, fellows, residents and physicians of the James L. Gordon Memorial Hospital;

WHEREAS, the association, which registration and incorporation with the Securities and Exchange Commission (SEC) is still on process, aims to give recognition to the creation of a Foundation that shall henceforth be solely and exclusively responsible for the collection, management, administration, disbursement and accounting of all amounts to be deducted from the professional fees of each member of the MSO-CARE;

WHEREAS, the funds shall be used by the MSO-CARE for the development of the Foundation and the community;

WHEREAS, all patients seeking assistance from the City shall be referred directly by the City to the MSO-CARE for screening, evaluation and approval of requests for discounts and other related matters;

WHEREAS, the Sangguniang Panlungsod Members deem that such agreement will pave way for the City to provide the Olongapo constituents with greater health benefits;

NOW, THEREFORE, on motion of Councilor Cynthia G. Cajudo, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the City Mayor to enter into a Memorandum of Agreement with Medical Staff of Olongapo-Consultants’ Assistance and Rehabilitation Endeavor (MSO-CARE) Foundation;

RESOLVED FURTHER, AS IT IS HEREBY FURTHER RESOLVED, that the signed Memorandum of Agreement (MOA) be subject for ratification by the Sangguniang Panlungsod.
