A RESOLUTION AUTHORIZING THE JAMES L. GORDON MEMORIAL HOSPITAL (JLGMH) REPRESENTED BY MAYOR JAMES GORDON, JR. TO ENTER INTO A MUTUAL ACCREDITATION AGREEMENT WITH
PRUDENTIAL LIFE HEALTH CARE REPRESENTED BY ITS AVP-MEDICAL DIVISION, DR. ALVIN T. CABRAL


WHEREAS, Prudential Health Care, Inc. a Corporation duly organized and is engaged in the promotion and marketing of its health care programs consisting of preventive health care, in-patient services, represented by its AVP-Medical Division, Dr. Alvin T. Cabral signifies their company’s intention to enter into a Mutual Accreditation Agreement with James L. Gordon Memorial Hospital (JLGMH) represented by Mayor James Gordon, Jr.;

WHEREAS, James L. Gordon Memorial Hospital (JLGMH) is the only public hospital in Olongapo City which provides excellent medical facility and health care;

WHEREAS, (JLGMH) is willing to accredit (PLHCI) as a preventive health care and provide medical assistance to its members/patients subject to certain terms and conditions;

NOW, THEREFORE, on motion of Councilor John Carlos G. delos Reyes, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the James L. Gordon Memorial Hospital (JLGMH) represented by Mayor James Gordon, Jr. to enter into a Mutual Accreditation Agreement with Prudential Life Health Care represented by its AVP-Medical Division, Dr. Alvin T. Cabral.
