A RESOLUTION AUTHORIZING THE HONORABLE CITY MAYOR ROLEN C. PAULINO TO REQUEST FOR AMBULANCE FOR JAMES L. GORDON MEMORIAL HOSPITAL (JLGMH) THROUGH THE AMBULANCE DONATION PROGRAM OF

THE PHILIPPINE CHARITY SWEEPSTAKES OFFICE (PCSO), AS WELL AS THE PROVISION FOR THE ALLOCATION OF NECESSSARY FUNDS FOR ITS UTILIZATION/OPERATION AND MAINTENANCE IN A MANNER CONSISTENT

WITH LAW

           

WHEREAS, the Philippine Charity Sweepstakes Office (PCSO) is a government agency which provides assistance to local government units (LGUs) and non-governmental organizations (NGOs) through fund augmentation to effect the delivery of primary health care services in the local community;

WHEREAS, James L. Gordon Memorial Hospital, under the Local Government Unit of Olongapo represented by the Honorable Mayor Rolen C. Paulino, endeavors to be one of the recipient of the requested ambulance;

WHEREAS, the PCSO’s Ambulance Donation Program is open to local government units operating government hospitals such as JLGMH;

WHEREAS, the provision for allocation of necessary funds for the utilization/operation and maintenance is required by the PCSO for the Ambulance Donation Program;

            NOW, THEREFORE, on motion of City Councilor Eduardo G. Guerrero, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

                 

RESOLVED, AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize as it hereby authorizes the Honorable City Mayor Rolen C. Paulino to request for ambulance for James L. Gordon Memorial Hospital (JLGMH) through the Ambulance Donation Program of the Philippine Charity Sweepstakes Office (PCSO), as well as provision for the allocation of necessary funds for its utilization/operation and maintenance in a manner consistent with law.

WHEREAS, IWC is engaged in soliciting enrollees (hereafter, referred to as “Member”) in its healthcare program, (hereafter, referred to as “Health Care”) consisting of out-patient, in-patient and preventive health care;

            WHEREAS, James L. Gordon Memorial Hospital accommodates health insurance providers which cater to patients who voluntarily avail of health insurance from private firms, either independent of or in addition to their Philippine Health Insurance (Philhealth);

WHEREAS, the hospital is willing to fully cooperate with IWC in the dispensation of needed medical services and facilitates to IWC in the dispensation of needed medical services and facilitates to IWC members under the terms and conditions set forth herein.

            NOW, THEREFORE, on motion of City Councilor Eduardo G. Guerrero, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

            RESOLVED, AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the Honorable City Mayor Rolen C. Paulino to enter into, execute, and sign For and on behalf  of James L. Gordon Memorial Hospital, the Memorandum of Agreement (MOA) with the IMS Wellth Care, Inc. (IWC), as regards to its Health Care Program in a manner consistent with law.

AND TECHNOLOGY (CELTECH COLLEGE), IN A MANNER

CONSISTENT WITH LAW

WHEREAS, Central Luzon College of Science and Technology (CELTECH College), is a duly recognized institution by the Commission on Higher Education (CHED) and Technical Education and Skills Development Authority (TESDA);

WHEREAS, James L. Gordon Memorial Hospital, is a tertiary hospital owned and managed by the City Government of Olongapo, existing pursuant to the laws of the Republic of the Philippines;

WHEREAS, CELTECH College respectfully requests the students of the Bachelor of Science in Nursing, Caregiver, Nursing Assistant and Midwifery Courses be admitted and allowed to undergo actual training at James L. Gordon Memorial Hospital and thereafter a certification be issued after completion of the required training;

WHEREAS, the agreement shall take effect on June 01, 2015 to June 30, 2017, and is deemed automatically renewed unless revoked jointly in writing by parties justifiable cause;

NOW, THEREFORE, on motion of City Councilors Benjamin G. Cajudo II and Eduardo G. Guerrero, with the unanimous accord of the Member of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the Honorable City Mayor Rolen C. Paulino to enter into, execute, and sign for and on behalf of James L.             WHEREAS, the Department of Health recognizes the participation of private sectors for quality of improvement in health system.

            WHEREAS, ASALUS Corporation/Intellicare is a health care specialist company providing and promoting comprehensive medical and health care, in-patient and out-patient services therein a\medical insurance policy holders can call for any inquiries during actual availment of their benefits and coordinating with medical health institutions to ensure delivery of health care services to its members;

            WHEREAS, James L. Gordon Memorial Hospital accommodates health insurance providers which cater to patients who voluntarily avail of health insurance from private firms, either independent of or in addition of their Philippine Health Insurance (Philheath);

            WHEREAS, the agreement would provide an additional safety mechanism that would assist patients and facilitate payment of their medical costs given that Intellicare has made an express warranty that it guarantees payment for all expenses incurred by its members for all medical care rendered by JLGMH including but not limited to emergency care, hospitalization by expenses, medicines, laboratory, and diagnostic procedures, doctors’ fees and other related care ordered by the attending accredited physician;

NOW, THEREFORE,  on motion of City Councilor Eduardo G. Guerrero, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

   

            RESOLVED, AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the Honorable City Mayor Rolen C. Paulino to enter into, execute, and sign for and on behalf of James L. Gordon Memorial Hospital (JLGMH) the Memorandum of Agreement with ASALUS Corporation/Intellicare, as regards to health care services to its members,  in a manner consistent with law.
