PRUDENTIAL LIFE HEALTH CARE REPRESENTED BY ITS AVP-MEDICAL DIVISION, DR. ALVIN T. CABRAL


WHEREAS, Prudential Health Care, Inc. a Corporation duly organized and is engaged in the promotion and marketing of its health care programs consisting of preventive health care, in-patient services, represented by its AVP-Medical Division, Dr. Alvin T. Cabral signifies their company’s intention to enter into a Mutual Accreditation Agreement with James L. Gordon Memorial Hospital (JLGMH) represented by Mayor James Gordon, Jr.;

WHEREAS, James L. Gordon Memorial Hospital (JLGMH) is the only public hospital in Olongapo City which provides excellent medical facility and health care;

WHEREAS, (JLGMH) is willing to accredit (PLHCI) as a preventive health care and provide medical assistance to its members/patients subject to certain terms and conditions;

NOW, THEREFORE, on motion of Councilor John Carlos G. delos Reyes, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the James L. Gordon Memorial Hospital (JLGMH) represented by Mayor James Gordon, Jr. to enter into a Mutual Accreditation Agreement with Prudential Life Health Care represented by its AVP-Medical Division, Dr. Alvin T. A RESOLUTION RATIFYING THE MEMORANDUM OF AGREEMENT ENTERED INTO BY AND BETWEEN THE PHILIPPINE NATIONAL POLICE CRIME LABORATORY (PNPCL) REPRESENTED BY POLICE CHIEF SUPT. ARTURO G. CACDAC, JR. AND THE CITY GOVERNMENT OF OLONGAPO REPRESENTED BY CITY MAYOR JAMES GORDON,JR.

WHEREAS, the City believes that the Police Force needs to be equipped with the best technology and forensic services in line with their investigative works, which are highly significant in the prompt solution and containment of crimes;

WHEREAS, the Sangguniang Panlungsod deems it imperative that the Philippine National Police Crime Laboratory (PNPCL) in the City of Olongapo can provide the technical support, scientific analysis, examination and findings of evidences needed by examiners in line with the investigations conducted for crime solution;

WHEREAS, an agreement forged between the Olongapo City Government and the PNPCL for the implementation as well as the assurance that such technical and forensic support are delivered is indeed of necessity;

NOW, THEREFORE, on motion of Councilor John Carlos G. Delos Reyes, with the unanimous accord of the members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED by the Members of the Sangguniang Panlungsod in session assembled to ratify, as it hereby ratifies the Memorandum of Agreement entered into by and between the Philippine National Police Crime Laboratory (PNPCL) represented by Police Chief Supt. MONATO, FOR THE USE OF A PARCEL OF LAND LOCATED AT THE SOUTH-WEST END OF EAST TAPINAC ELEMENTARY SCHOOL, FOR JAMES L. GORDON COUNCIL-BSP HEADQUARTERS

WHEREAS, Sangguniang Panlungsod passed Resolution No. 183, Series of 2006, approving the proposed Boy Scouts of the Philippines (BSP)-James L. Gordon Council Building as one of the priority projects of the City for the calendar year 2006;

WHEREAS, BSP-James L. Gordon Council passed Resolution No. 07, Series of 2007 requesting Senator Richard J. Gordon to support the BSP-James L. Gordon Council Headquarter project and Resolution No. 03, Series of 2007 designating Dr. Ligaya B. Monato as signatory to the proposed Memorandum of Agreement for the use of the subject parcel of land;

WHEREAS, a Memorandum of Agreement for the use of the said parcel of land is necessary for the project to commence;

WHEREAS, the Sangguniang Panlungsod during in its 9th regular session deliberations agreed to grant the Boy Scouts of the Philippines, for an initial period of fifteen years and renewable thereafter, the use of a parcel of land with an area consisting of approximately 408 square meters located at the South-West-end of East Tapinac Elementary School, bounded by East Tapinac Oval Track Grandstand at SOUTH, Bacon St. at the WEST, East Tapinac Oval Track at EAST, and East Tapinac Elementary School building at NORTH;

NOW, THEREFORE, on motion of Councilor Edwin J. Piano and with the unanimous accord of the Members of Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the City Mayor James Gordon, Jr. to enter into Memorandum of Agreement with BSP-James L. Gordon Council represented by Dr. Ligaya B. Monato, for the use of a parcel of land located at the South-West end of Tapinac Elementary School, for James L. Gordon Council-BSP Headquarters.

TO ENTER INTO A MEMORANDUM OF AGREEMENT FOR AND IN BEHALF OF THE CITY HEALTH DEPARTMENT OF OLONGAPO WITH GLOBAL CITY INNOVATIVE COLLEGE SUBIC INTERNATIONAL INCORPORATED (GCICSII) REPRESENTED BY CORAZON B. GUANCO, VICE PRESIDENT FOR ADMINISTRATION

WHEREAS, Global City Innovative College Subic International Incorporated (GCICSII), an educational institution located at Zambales Highway, Upper Cubi Subic Bay Freeport Zone, aims to uplift the health and well-being of the people, which complements the commitment of the City Health Office in its mandate to provide basic health services to its people within its jurisdiction;

WHEREAS, both has committed to address the needs of the people of Olongapo City attested through the Memorandum of Agreement signed by both parties which states the request for affiliation and training of students of Global City Innovative College Subic International Incorporated (GCICSII) to the different Rural Health Units of Olongapo City. The said agreement also states the prohibition that the GCICSII complies with the existing laws as stated by the Department of Health Administrative Order 5-A, Series of 1996 and other related laws and orders pertaining to Related Learning Experience of the students;

WHEREAS, the said Memorandum of Agreement (MOA) shall be the basis for the function, responsibilities and other arrangements of the City Health Department and GCICSII in their joint effort to address the immediate health needs of the people of Olongapo City;

WHEREAS, the affiliation and training fee set by the Administrative Order 5-A series of 1996 of the Department of Health shall be the minimum fees/ charges to be shouldered by the GCICSII and shall be included in the MOA;


NOW, THEREFORE, on joint motion of Councilors Marey Beth D, Marzan, and Angelito W. Baloy, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in the session assembled, to authorize, as it hereby authorizes the City Mayor James Gordon, Jr. to enter into a Memorandum Of Agreement (MOA) for and in behalf of the City Health Department of Olongapo with Global City Innovative College Subic International Incorporated (GCICSII) represented by Corazon B. A RESOLUTION EXPRESSING DEEPEST CONDOLENCES TO THE BEREAVED FAMILY OF COUNCILMAN FEDERICO G. VILLEGAS, JR.OF BARANGAY KALAKLAN

WHEREAS, Hon. Federico G. Villegas, Jr., the number one Kagawad of Barangay Kalaklan for almost nine (9) years, peacefully joined his Creator on April 19, 2007;

WHEREAS, one of the most loved and respected Councilman in the community, Kagawad Villegas served as Barangay Secretary before winning the electoral battle and was known to be the right hand of the Chairman in the performance of his functions and projects;

WHEREAS, Barangay Kalaklan will never forget his keen sense of understanding on significant matters and handling of complaints as well as his wise counsel and concern to his constituents;

WHEREAS, Olongapo acknowledges Kagawad Villegas’ leadership, dedication and devotion to public service and joins his family in the loss of their beacon and a doting Tatay to his grandchildren;

NOW, THEREFORE, with the unanimous accord of the Members of Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to express, as it hereby expresses sorrow over the passing away of Hon. Federico G. Villegas, Jr. and condoles with his bereaved family.

Let a copy of this Resolution be furnished the bereaved family of Hon. Federico G. Villegas, Jr. for their information and record.
