A RESOLUTION EXPRESSING ITS DEEPEST CONDOLENCES TO THE

 BEREAVED FAMILY OF THE LATE RUTH FABAY RIVERA, SECRETARY

 OF BARANGAY NEW ASINAN. OLONGAPO CITY

WHEREAS, Ruth Fabay Rivera, as Barangay Secretary of  Barangay  Asinan,  Olongapo City died on August 16, 2015

WHEREAS,  her untimely death has caused sadness to the people of Barangay Asinan and to this august body as well;

WHEREAS, Ruth Fabay Rivera served Barangay Asinan as Secretary for more than twenty three (23) years with utmost competence and integrity, working above and beyond the call duty;

WHEREAS, as a barangay employee who served her constituents well, the people of Olongapo City condoles that another dedicated public servant passed away and so deeply share in the sorrow of her bereaved family;

WHEREAS, despite all the heartaches and grief due to her sudden death, this august body representing the Olongapeños takes pride of her services as barangay secretary;

NOW, THEREFORE,  with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to express, as it hereby expresses its deepest condolences to the bereaved family of the late Ruth Fabay Rivera, Secretary of Barangay New Asinan,  Olongapo City.

A RESOLUTION EXPRESSING DEEPEST CONDOLENCES TO THE BEREAVED FAMILY OF THE LATE EDGARDO ZAMORA TALLADA, A.K.A. TATA EDDIE, A LEGENDARY MUSICIAN OF OLONGAPO CITY AND FORMER COUNCILMAN

OF BARANGAY MABAYUAN

            WHEREAS, Edgardo “Tata Eddie” Tallada, who was born on August 15, 1939 in Bambang, Sta. Cruz, Manila, is a legendary icon of Olongapo City’s Music Entertainment;

            WHEREAS, he died in the Grace of Our Lord, on August 8, 2015 at the age of 75, leaving his wife, Evangeline Palma-Tallada, three children Edlyn, Edgardo Jr. and Erlinda in utmost grief and sorrow;

            WHEREAS, he served as a Barangay Councilman of Barangay Mabayuan in 1984 and continued to serve his community, in and out public office;

            WHEREAS, his career  as a musician called as “combo”, started in the 1950’s;

            WHEREAS, in 1981, he founded the “Eddie Tallada and Company”, a band performing Oldies but Goodies music that became very popular in the music industry and put Olongapo City in the map of Philippine entertainment;

            WHEREAS, he was the recipient of the 1989 Aliw Awards Ambassador for Music;

            WHEREAS, his great dedication keeping the Oldies but Goodies music alive in Olongapo City for so many years, became his legacy as a great musician for all seasons.

            NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

    

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to express as it hereby expresses deepest condolences to the bereaved family of the late Edgardo Zamora Tallada, a.k.a. A RESOLUTION EXTENDING DEEP SYMPATHY AND CONDOLENCES TO THE BEREAVED FAMILY OF THE LATE PAULINO T. CRUZ, WHO PEACEFULLY

JOINED HIS CREATOR

WHEREAS, Mr. Paulino T. Cruz peacefully joined his creator last July10, 2015 at the age of 78, leaving behind his beloved wife and five children;

WHEREAS, his extended family and friends at the City Government of Olongapo, whom he has served as a government employee, a volunteer and as a civic servant, do not only mourn his passing away but join his family in praying for the eternal repose of his soul;

WHEREAS, “Tata Paul” as he is fondly called by his family and friends, is acknowledged for his various significant involvement and achievement that helped shape the history of Olongapo;

WHEREAS, Tata Paul is one of the first volunteers who helped in the transition of the former U.S. Naval Facility into commercial use; from planning to lobbying in the Senate and through the period of protecting and preserving the facilities left behind by the US Navy until its successful conversion into the first Freeport of the Philippines;

WHEREAS, Tata Paul has contributed his lifetime for the progress of Olongapo and wherever he may have worked. He was one of the pioneers of Gordon College, holds one of the highest positions for Filipinos working inside the US Naval Facility, author of the SBMA Seaport Manual and an esteemed civic leader, being the Past Grand Knight of the Knights of Columbus;

WHEREAS, during his lifetime, he was not only a devoted husband, a loving father, an exemplary employee, a dedicated volunteer, a prime mover, and an active civic leader, but he had been a man who inspired people from all walks of life;  

NOW, THEREFORE,  with the unanimous accord of the Members of the Sagguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED,  by the Sangguniang Panlungsod in session assembled,  to extend as it hereby extends deep sympathy and condolences to the  bereaved family of the late Paulino T. Cruz, who peacefully joined his Creator.

A RESOLUTION EXTENDING SINCEREST AND HEARTFELT CONDOLENCES

TO THE BEREAVED FAMILY OF FORMER OLONGAPO CITY ACCOUNTANT DENNIS R. MARTINEZ WHO PEACEFULLY JOINED HIS CREATOR ON JUNE 1, 2015

WHEREAS, in the afternoon of June 1, 2015, a day of grief and sorrow enveloped the hearts of many, as a good son of the City of Olongapo, former City Accountant Dennis R. Martinez peacefully joined his creator at a young age of 47.

WHEREAS, Dennis Martinez dutifully served the Olongapo City Government for almost twenty-six years, starting in June 1988 up to March 2014. He gradually rose from the ranks, and finally held the position as the City Accountant from 1998 up to the first quarter of 2014;

WHEREAS, with his public service initiatives, and as an active member of the city’s finance team, he was commended as the Philippine Institute of Certified Public Accountants (PICPA) Young Achievers Awardee For Government Service in 1993 at Davao City;

WHEREAS, his track record in school speaks of his brilliance. Dennis as he is fondly called, was a scholarship grantee of the James Gordon Scholarship Foundation and graduated with a degree of Bachelor of Science in Mathematics at the University of the Philippines, Los Baños in 1988.

Immediately after graduating, he started working at the Olongapo City Government. He then took up Bachelor of Science in Accountancy at Columban College, juggling his time as working student and as a family man, but still excelled in class and managed to graduate with honors;

  WHEREAS, Dennis was survived by his wife Rozzine and four children namely Dennice Rose-24, Dionne Roze-20, Dennis Richard-19, and Dennis Raphael-11, who regard him as an ideal and loving husband, a good provider and a dedicated father. As an only child, his mother Regina Romero is filled with pride and joy of having a son like him;

WHEREAS, known to be a reserved man, he was a thoughtful and warm person to his friends, who had simple joys and needs;

WHEREAS, his demise is not only a personal grief for his bereaved family but also for the City of Olongapo and its people whom he served his entire career life;

NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to extend, as it hereby extends sincerest and heartfelt condolences to the bereaved family of former City Accountant Mr. Dennis R. Martinez who peacefully joined his Creator on June 1, 2015.
