A RESOLUTION AUTHORIZING THE CITY MAYOR, HONORABLE ROLEN C. PAULINO TO ENTER INTO AND SIGN THE MEMORANDUM OF AGREEMENT WITH ‘SATURDAY AFTERNOON GENTLEMEN SAILING’ (SAGS) REPRESENTED BY  ITS CHAIRMAN, MR.  FEDERICO SANDOVAL, AS REGARDS THE HOLDING OF THE ‘SUBIC BAY TO BORACAY RACE’ (SBBR) ON FEBRUARY 27, 2015 UP TO MARCH 2, 2015 AND THE ‘BORACAY CUP REGATTA (BCR)’ ON MARCH 2-6 2015, WITHIN THE SUBIC BAY FREEPORT ZONE AND THE CITY OF OLONGAPO

WHEREAS, Saturday Afternoon Gentlemen Sailing  (SAGS),the leading organizer of local and international yacht racing event  in the country, wrote a letter of proposal dated February 3, 2015 to the City Mayor Rolen C. Paulino asking for the City’s cooperation and support to the upcoming Standard Insurance ‘6^th Subic Bay to Boracay Race (SBBR)’ on February 27, 2015 to March 2, 2015 to the ‘Boracay Cup Regatta (BCR)’ on March 2-6, 2015 and to the ‘8^th Commodores Cup Regatta’ on April 5-11, 2015;

WHEREAS, the said event are expected to draw a number of sailing enthusiasts and the City of Olongapo was invited to be part of the 2015 International Regatta to promote the Philippines, particularly the Subic Bay and surrounding areas as a world class tourist destination, domestically and internationally, that offers exciting, fun, and diverse experience in the water and land natural resources of Olongapo and Subic Bay;

WHEREAS, a Memorandum of Agreement is deemed necessary to carry out the specified mutual covenants between the City of Olongapo and the SAGS.

NOW, THEREFORE, on joint motion of City Councilors Aquilino Y. Cortez, Jr.  Emerito Linus D. Bacay and Eduardo G. Guerrero, with the unanimous accord of the Members of Sanggunian Panlungsod present;

RESOLVED, AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to authorize, as it hereby authorizes  the City Mayor Rolen C. Paulino to enter into and sign the Memorandum of Agreement with ‘Saturday Afternoon Gentlemen Sailing’ represented by  its Chairman, Mr.  Federico Sandoval as regards the holding of the ‘Subic Bay to Boracay Race’ (SBBR) on February 27, 2015  up to March 2, 2015 and the ‘Boracay Cup Regatta’ (BCR) on March 2-6 2015, within the Subic Bay Freeport Zone and the City of Olongapo.

Let a copy of the signed Memorandum of Agreement (MOA)  be made an integral part of this resolution.

GORDON MEMORIAL HOSPITAL WITH THE DEPARTMENT OF HEALTH – CENTER FOR HEALTH DEVELOPMENT 3 REGARDING THE PROVISION OF ASSISTANCE FOR INDIGENT PATIENTS THROUGH THE MEDICAL HEALTH CARE ASSISTANCE PROGRAM (MHCAP)

WHEREAS, the Medical Health Care Assistance Program (MHCAP), under the Department of Health, aims to provide assistance to indigent patients admitted in government heath facilities;

WHEREAS, the status of the implementation of the said program in Olongapo City has been received from the DOH Regional Office 3, which confirms the allocation  of additional funding/budget for indigent patients seeking assistance for their consultation or admission to the City-run hospital in the amount of Two Million Pesos (PhP2,000,000.00)

WHEREAS, to ensure a more effective and efficient system for managing and monitoring the MHCAP  and a regular replenishment process for fund allocation to DOH-RO III and reimbursement process for LGU – managed hospitals, a Supplemental Memorandum of Agreement is in order;

NOW, THEREFORE, on motion of City Councilor Eduardo G. Guerrero. with the unanimous accord of the Members of Sangguniang Panlungsod present;

RESOLVED, AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to authorize, as it hereby authorizes  the City Mayor,  Hon. Rolen C. Paulino, to enter into and sign the Supplemental Memorandum of Agreement, for and on behalf of the James L. Gordon Memorial Hospital with the Department of Health – Center for Health Development 3 regarding the provision of assistance for indigent patients through the Medical Health Care Assistance Program (MHCAP).

A RESOLUTION AUTHORIZING THE CITY MAYOR, HON. ROLEN C. PAULINO TO SIGN AND ENTER INTO A CONTRACT OF AFFILIATION, FOR AND ON BEHALF OF THE GORDON COLLEGE, THIS CITY WITH SAN LAZARO HOSPITAL REPRESENTED BY DR. WINSTON S. GO RELATIVE TO THE TRAINING OF NURSING STUDENTS

WHEREAS, the Gordon College is a tertiary level learning institution duly authorized and recognized by the Commission on Higher Education to accept enrollees for Health Science and related programs;

WHEREAS, the pressing demand for health professionals in the global market led to the growing number of enrollees to the said courses and a tertiary-care capable hospital is necessary in complying to the Nursing Act of 2002 that requires nursing students to undergo Related Learning Experience (duty hours) in the community as well as in hospital setting;

WHEREAS, the San Lazaro Hospital is an tertiary hospital that has the capability and the facilities to impart the real hospital set up and exposures required  of a nursing job to the students;

WHEREAS, San Lazaro Hospital agrees and allows the affiliation and training of nursing students of the Gordon College subject to the policies, procedures and guidelines  governing  Affiliation and Training of Students prescribed by the Department of Health/Hospital/Rural Health Units and other Health Agencies;

NOW, THEREFORE, on motion of City Councilor Eduardo G. Guerrero. with the unanimous accord of the Members of Sangguniang Panlungsod present;

RESOLVED, AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to authorize, as it hereby authorizes  the City Mayor,  Hon. Rolen C.
