A RESOLUTION EXTENDING OLONGAPO CITY’S SYMPATHY AND SOBREITY OVER THE DEMISE OF MR. JOHNNY BLAZA CHOA, FORMER CITY BUDGET OFFICER, WHO JOINED OUR CREATOR LAST 06 JANUARY 2011

WHEREAS, Mr. Johnny Blaza Choa, born on 03 February 1947 at Benavidez Street, Tondo, City of Manila, peacefully joined our creator last 06 February 2011 at the age of 63. He is survived by his family with his bereaved wife Rizalina Familara Choa and son John;

WHEREAS, Mr. Choa served as City Budget Officer from 16 October 1980 to 31 December 2008,a total of 29 fruitful and dedicated years of service;

WHEREAS, “Johnny/Pangyaw”, as he was fondly called by colleagues, subordinates and fellow government employees, manifested enthusiasm and dynamic energy towards performing his daily tasks that helped in the attainment of the City’s goal and objectives;

WHEREAS, his loss is mourned not only by his family, but by the City of Olongapo as well, for his being an inspiration in its quest for continued excellence and competence;

WHEREAS, his presence may not be physically felt but his memories would always remain in our hearts and the thought that in a part of our history is engraved a person by the name of Johnny Blaza Choa who was once with us in shaping Olongapo City;

NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to extend, as it hereby extends Olongapo City’s sympathy and sobriety over the demise of Mr. Johnny Blaza Choa, former City Budget Officer, who joined our Creator last 06 January 2011.

Let copies of this resolution be presented to his family represented by his wife Rizalina Familara-Choa for their posterity.

In the interest of public service and on motion of City Councilors Gina Gulanes-Perez and co-sponsored by City Councilors Rodel S. Cerezo, Eyrma Yvette Marzan-Estralla, James G. Delos Reyes, Elena C. Dabu, Eduardo J. Piano, Edna A. Elane and Dolly Mae M. Ramos, with the majority decision of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to enact the following Ordinance.

ORDINANCE NO. 01
(Series of 2011)

AN ORDINANCE AUTHORIZING THE ANNUAL BUDGET OF THE CITY OF OLONGAPO FOR FISCAL YEAR 2011 IN THE AMOUNT OF TWO BILLION THREE HUNDRED SEVENTY MILLION FOUR HUNDRED ELEVEN THOUSAND THREE HUNDRED SEVENTEEN PESOS (PHP 2,370,411,317.00) COVERING VARIOUS EXPENDITURES FOR THE OPERATION OF THE CITY GOVERNMENT FOR FISCAL YEAR 2011, AND APPROPRIATING THE NECESSARY FUNDS FOR THE PURPOSE

BE IT ORDAINED, BY THE SANGGUNIANG PANLUNGSOD IN SESSION ASSEMBLED, THAT:

SECTION 1. The Annual Budget of the City of Olongapo for Fiscal Year 2011 in the total amount of Two Billion Three Hundred Seventy Million Four Hundred Eleven Thousand Three Hundred Seventeen Pesos (P2,370,411,317.00) covering the various expenditures for the operation of the City Government for the year 2011 is hereby approved.

The budget documents consisting of the following are incorporated herein and made integral part of this Ordinance:

1. Budget of Expenditures and Source of Financing
2. Annual Investment Program

SECTION 2. Sources of Funds. (Please refer to Local Expenditure Program for details)

SECTION 3. Use of Funds. (Please refer to Local Expenditure Program for details)


SECTION 4. Authority to Grant the 100% Full Implementation of First Tranche Monthly Salary Schedule. Authority is hereby given to grant the 100% Full Implementation of First Tranche Monthly Salary Schedule to the City Government Personnel effective January 1. 2011.

SECTION 5. Use of Savings and Augmentation. In accordance with Section 336 of Republic Act No. 7160, the Local Government Code of 1991, the Mayor is authorized to augment any item in the approved annual budget for their respective offices from savings in other items within the same expenses class of their respective appropriations.

For this purpose, “savings” refer to portions or balances of any programmed appropriation free from any obligation of encumbrance, still available after the satisfactory completion or the unavoidable discontinuance or abandonment of the work, activity or purpose for which the appropriation is authorized, or arising from unpaid compensation and related costs pertaining to vacant positions and leaves of absence without pay.

“Augmentation” implies the existence in the budget of an item, project, activity or purpose with an appropriation which, upon implementation or subsequent evaluation of needed resources, is determined to be deficient.

SECTION 6. Priority in the Use of Personal Services Savings. Priority shall be given to the personnel benefits of local employees in the use of Personal Services savings.

SECTION 7. Unexpended Local Disaster Risk Reduction and Management Fund (LDRRMF) shall accrue to a special trust fund solely for the purpose of supporting disaster risk reduction and management activities of LDRRMCs within the next five (5) years. Any such amount still not fully utilized after five (5) years shall revert back to the general fund and made available for other social services to be identified by the Sangguniang Panlungsod pursuant to Rule 18, Section 3 of R.A. 10121.

SECTION 8. Authority to charge previous years accounts to the current year. Authority is hereby given to charge to the CY 2010 savings, the previous year’s unpaid services/goods that was actually rendered/received.

SECTION 9. Separability Clause. If, for any reason, any Section or provision of this Appropriation Ordinance is disallowed in Budget Review or declared invalid by proper authorities, other Sections or provisions hereof that are not affected thereby shall continue to be in full force and effect.

SECTION 10. Effectivity. The provisions of this Appropriation Ordinance shall take effect upon its approval except for the salary adjustment which shall take effect on January 1, 2011.

A RESOLUTION AUTHORIZING THE CITY MAYOR, JAMES GORDON JR. TO ENTER INTO A MEMORANDUM OF AGREEMENT BETWEEN JAMES L. GORDON MEMORIAL HOSPITAL AND THE INTERNATIONAL STANDARDS CERTIFICATION PHILIPPINES, INC.

WHEREAS, the International Standards Certification Philippines, Inc. is a Certifying Body of ISO which audits the hospital’s Certification System, reviews and ensures compliance with selected standards and confirms the competencies required and the management system of the James L. Gordon Memorial Hospital;

WHEREAS, upon the completion of the certification process and the issuance of a certificate, it will the passing of the standard and provide high quality service and give the hospital some degree of prestige of catering to patients from companies and locators of SBMA as well as its international clients;

NOW, THEREFORE, on motion of City Councilor Edna A. Elane, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the City Mayor , James Gordon Jr. to enter into a Memorandum of Agreement between James L. Gordon Memorial Hospital and the International Standards Certification Philippines, Inc.

RESOLUTION NO. 02

A RESOLUTION AUTHORIZING THE CITY MAYOR TO ENTER INTO A MEMORANDUM OF AGREEMENT WITH THE PHILIPPINE INSTITUTE OF VOLCANOLOGY AND SEISMOLOGY-DEPARTMENT OF SCIENCE AND TECHNOLOGY (PHILVOCS-DOST) FOR THE PROVISION OF RAPID EARTHQUAKE DAMAGE ASSESSMENT SYSTEM (REDAS) SOFTWARE, TRAINING AND TECHNICAL SUPPORT TO THE CITY OF OLONGAPO


WHEREAS, the Philippine Institute of Volcanology and Seismology – Department of Science and Technology (PHILVOCS-DOST), a government agency, created under existing laws of the Republic of the Philippines, represented by Director Renato U. Solidum, Jr., is the agency mandated to monitor volcanic activities and earthquake occurrences and conduct studies leading to volcanic eruption mitigation and earthquake risk reduction;

WHEREAS, PHILVOCS-DOST developed a software called Rapid Earthquake Damage Assessment System (REDAS) that aims to produce seismic hazard and risk maps, minutes after the occurrence of a potentially damaging earthquake and can be used as a tool in mainstreaming disaster risk reduction into the local development planning process;

WHEREAS, Iba Fault, Manila Trench, Philippine Trench and Mount Pinatubo are some of the active contributors of earthquake that are in close proximity to Olongapo City;

WHEREAS, PHILVOCS-DOST would like to share the benefits of REDAS to the Philippine communities including the city Government of Olongapo by providing the software, training, technical support and future upgrade of the REDAS software;

WHEREAS, a signed Memorandum of Agreement between the City and PHILVOCS-DOST is compulsory prior to the provision of REDAS software, training and technical support;

NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the City Mayor to enter into a Memorandum of Agreement with the Philippine Institute of Volcanology and Seismology – Department of Science and Technology (PHILVOCS-DOST) for the provision of REDAS software, training and technical support of the City of Olongapo.
