TO THE BEREAVED FAMILY OF HONORABLE CITY COUNCILOR AQUILINO Y. CORTEZ, JR. FOR THE DEATH OF THEIR PATRIARCH, MR. AQUILINO O. CORTEZ, SR. WHO PEACEFULLY JOINED HIS CREATOR ON MAY 06, 2015 AT THE AGE OF 76

WHEREAS, on May 06, 2015, the father of Honorable Councilor Aquilino Y. Cortez, Jr., Mr. Aquilino O. Ocampo Cortez, Sr. peacefully joined his creator at the age of 76. Without doubt, this date marks a lowest point in the life of City Councilor Aquilino Y. Cortez, Jr. and his family wherein the company of relatives, friends and colleagues and the consoling words and sincere expression of sympathies from others who are condoling with them are their only refuge;

WHEREAS, Mr. Aquilino O. Cortez, Sr. may have lived his life to the fullest and died at a relatively old age which for many is both a blessing and cause to celebrate life. But despite all the comforting thoughts, his loss is surely painful to his beloved family;

WHEREAS, in their grief, may this Resolution of sympathies and condolences may somehow ease the sorrow of the family and help them to immediately come to terms with the reality of losing someone so dear;

NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

 DR. FE EVANGELINE ELA-LIM

WHEREAS, Dr. Fe Evangeline Ela-Lim, M.D., peacefully joined our Creator on April 26, 2015 at the age of 57;

WHEREAS, “Dra. Lim”, as she is known to many, has committed herself to community service by rendering professional services to many of the Barangay Health Centers in Olongapo City as an Obstetrician-Gynecologist  at the City Health Department for eight (8) years, since 01 December 2007;

WHEREAS, she worked as a Rural Health Physician of Barangay Kalalake Health Center and was an active member of the Olongapo City Medical Society. As a Rural Health Physician, she has shown genuine concern for her patients, gaining the respect and administration of her colleagues, as well as the people whom she has served with utmost patience and dedication;  

WHEREAS, the Sangguniang Panlungsod shares the grief of the Lim family for having lost a remarkable woman, a mother, a physician and a friend;

NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to express as it hereby expresses its most profound sorrow and extends deepest condolences to the bereaved family of the late Dr. Fe Evangeline Ela-Lim.

A RESOLUTION EXPRESSING DEEPEST CONDOLENCES TO THE BEREAVED FAMILY OF THE LATE HON. JANTZEN ROQUE ARCALAS, FORMER SANGGUNIANG KABATAAN (SK) CHAIRMAN OF BARANGAY NEW KALALAKE

WHEREAS, Hon. Jantzen R. Arcalas, 39 years old and born on October 4,1975 is the former Sangguniang Kabataan Chairman of Barangay Kalalake, Olongapo City;

WHEREAS, he died in the Grace of our Lord on April 15, 2015, leaving his wife Jerichelle Arcalas, two children Hanes Yuriel Arcalas and Hanna Eriyelle Arcalas, and his parents Benjamin Arcalas and Alicia Roque in utmost grief and sorrow;  

WHEREAS, Jantzen, as fondly known to many, was an active youth leader who served as Sangguniang Kabataan Chairman of Barangay New Kalalake for five (5) years from 1996 to 2002 and served as a former member of Civilian Security Service Unit (CSSU) for two (2) years;

WHEREAS, during his term as Sangguniang Kabataan Chairman, he had shown unblemished dedication to the community which he served wholeheartedly;

WHEREAS, true to his leadership position, he emphasized the rights and capabilities of the youth and participated in activities and supported the various programs and projects of Barangay New Kalalake and the City of Olongapo;

WHEREAS, the family and friends of Jantzen considered his demise a great loss and he will be missed so dearly.

NOW, THEREFORE, on motion of City Councilor Randy C. Sionzon, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

A RESOLUTION EXPRESSING DEEPEST SYMPATHIES TO THE BEREAVED

FAMILY OF DAN C. ERALDO A.K.A. DJ DAN AND COMMENDING THE LATTER’S SIGNIFICANT CONTRIBUTIONS TO THE LOCAL BROADCASTING INDUSTRY

WHEREAS, Dan C. Eraldo popularly known as “DJ DAN” was a radio announcer of DWGO AM 1008 KHZ back in early 1990s where he handled music programs that brought rhythm in every home reached by the airwaves. His introduction of greetings and song dedications alongside with music then created a perfect form of entertainment to the subtle broadcast industry in Olongapo;

WHEREAS, in late 1990s, Subic Broadcasting Corporation (SBC) established DWOK FM 97.5 and DJ Dan was one of the FM station’s disc jockeys who pioneered the station’s music and greetings format;

WHEREAS, since DJ Dan possessed a remarkably powerful deep manly voice, he captured the listening public’s attention which made people glue their ears with radio sets every time he uses the microphone and be on air; Moreover, DJ Dan could play with his voice to sound like there were several characters which created animation to his programming and radio commercials;

WHEREAS, inherent to his personality was the effortless way of making people laugh  in his positive or negative commentaries, be it in the form of pure comedy or at times with slight embarrassment caused to the  listeners, but  nevertheless, DJ Dan undoubtedly made  everyone happy in every word he uttered;

WHEREAS, due to his exceptional people skills and convincing power, he was  appointed in sales and marketing where he was able to easily tap advertisers and sponsors which resulted to good revenue to the radio station;

WHEREAS, DJ DAN proceeded to organizing events like concerts, competition and his renowned ‘Stage Slammin”- a group dance contest which had drawn many young members in the community to shift to dancing and bring out their unrecognized talents in motion and rhythm;

WHEREAS, dubbed as Olongapo’s “Master Showman”, DJ Dan discovered many local talents with exceptional skills in singing, dancing, modelling among many others. He likewise created a young group of trainees he called “DJs in a Row”, where he trained the members in radio announcing, event hosting and personality development;

WHEREAS, Dan C. Eraldo, a young vibrant broadcaster and entertainer who created an impact to the society, truly deserves a tribute for each and every fragment of his contribution during the golden years of local media;

NOW THEREFORE, on motion of Councilor Eduardo J. Piano, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

RESOLVE, AS IT IS HEREBY RESOLVED, by the SangguniangPanlungsod in session assembled to express deepest sympathies to the bereaved family of Dan C. Eraldo a.k.a. DJ Dan and commending the latter’s significant contributions to the Local Broadcasting Industry;

A RESOLUTION EXPRESSING DEEPEST CONDOLENCE AND SYMPATHY TO THE VIZCOCHO FAMILY, BEREAVED WITH THE PASSING AWAY OF THEIR PATRIARCH, MR. VICTORIO S. VIZCOCHO, SR.


WHEREAS, Mr. Victorio Siat Vizcocho, Sr., one of the original residents of Olongapo City, returned to his creator on October 25, 2007 at the age of 79;

WHEREAS, during his teens, a patriot at his young age, he served as “runner” for guerillas in Sourthern Zambales during the Japanese occupation thereby, aiding the success of operations and safety of guerillas and Filipino civilians, alike;

WHEREAS, after the second world war, he served as a low key worker in the rehabilitation and reconstruction of Olongapo;

WHEREAS, “Vic Sr.” or “Manong Turing” as he is fondly called, worked as a civilian employee of the United States Navy from 1958 to 1979, rising from the ranks to Foreman of Transportation Department, Public Works Center, U.S. Navy, Subic Bay;

WHEREAS, aside from the numerous awards and commendations, he was a consistent recipient of the Sustained Superior Performance (SSP) Award, where, he manifested the inherent trait of the Filipinos’ will and determination to excel in any field of endeavor;

WHEREAS, after his retirement from employment as a civilian employee of the US government, “ManongTuring” engaged into construction and realty business;

WHEREAS, as a family man, he was a good father who instilled in his children’s minds the importance of the family as the basic unit of society. He raised all six children: five sons and an only daughter (the youngest), in accordance with accepted morals and practices of society and Christian spirituality;

WHEREAS, as a full blooded Olongapeño he spoke at every opportunity of the beauty and glory of Olongapo since it was a barrio of Subic town, a US Naval Reservation, a municipality until it became a city;

NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to express, as it hereby expresses deepest condolence and sympathy to the Vizcocho family, bereaved with the passing away of their Patriarch, Mr. Victorio S. Vizcocho, Sr.
