A RESOLUTION AUTHORIZING THE CITY MAYOR, HON. JAMES GORDON, JR. TO ENTER INTO, EXECUTE AND SIGN FOR AND IN BEHALF OF THE JAMES L. GORDON MEMORIAL HOSPITAL (JLGMH) A MEMORANDUM OF AGREEMENT (MOA) WITH THE ASSISTANCE ALLIANCE INTERNATIONAL (AAI) INC.

WHEREAS, in an effort to increase its income, the JLGMH is encouraging the accreditation of different Health Maintenance Organization (HMO) who can pay the hospital for the confinement of their members;

WHEREAS as a partner, the Assistance Alliance International (AAI) is in favor to post Two Hundred Thousand Pesos (Php200,000.00) Surety Bond to guarantee payment of all obligations;

NOW, THEREFORE, on motion of City Councilor Edna A. Elane, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes, the City Mayor, Hon. James Gordon, Jr. to enter into, execute and sign for and in behalf of the James L. Gordon Memorial Hospital (JLGMH) a Memorandum of Agreement (MOA) with the Assistance International Inc.

A RESOLUTION AUTHORIZING THE CITY MAYOR, HONORABLE JAMES GORDON JR., TO ENTER INTO A MUTUAL ACCREDITATION AGREEMENT FOR AND ON BEHALF OF THE JAMES L.GORDON MEMORIAL HOSPITAL WITH THE BLUE CROSS HEALTH CARE, INC. REPRESENTED BY ITS MEDICAL DIRECTOR, NOEL L. ROSAS, MD,FPCP,FPCC

WHEREAS, Blue Cross Health Care, Inc., is a company engaged in the promotion and marketing of its health care programs consisting of preventive health care, in-patient and out-patient services;

WHEREAS, the James L. Gordon Memorial Hospital is a tertiary medical facility owned by the local government of Olongapo City;

WHEREAS, the aforesaid company can only accomplish its tasks through its accreditation to a hospital that provides excellent medical facility and quality care, such as the James L. Gordon Hospital;

WHEREAS, the City deems to authorize the City Mayor for and on behalf of the James L. Gordon Hospital to enter into an agreement with Blue Cross Health Care, Inc., regarding their health care package;

NOW, THEREFORE, on motion of City Councilor Aquilino Y. Cortez, Jr. with the unanimous accord of the members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to authorize, as it hereby authorizes the City Mayor, Honorable James Gordon Jr., to enter into a Mutual Accreditation Agreement for and on behalf of the James L.Gordon Memorial Hospital with the Blue Cross Health Care, Inc., represented by its Medical Director, Noel L. Rosas, MD,FPCP,FPCC.
