RESOLUTION NO. 05

A RESOLUTION DECLARING A MEASLES OUTBREAK IN OLONGAPO CITY

WHEREAS,  the Department of Health Assistant Secretary Dr. Eric Tayag, who is also the Director of the National Epidemiology Center, said  on a news  report dated  January 3, 2014,  that there were already 1,724 confirmed measles cases nationwide, from  the period January 1 to December 14, 2013 where 21 cases led to mortality.  He added that  out of the  seventeen (17) regions in the country, only four (4) regions did not report an increase in the number of measles cases;

WHEREAS,  Regional Epidemiologist, Dr. Jessie Fantone, in his visit to the City of Olongapo January 14, 2014 assessed the situation on the spread of the said viral infection and gave a verbal recommendation to declare an outbreak after his review of the cases in the City;

WHEREAS,  62  cases of  measles, in a two-week period, from January 1 to 14, 2014,  were recorded as compared with  only 23 total number of cases for CY 2013;

WHEREAS, due to  confirmed cases and high incidence in neighboring provinces,  Dr. Jessie Fantone  recommended the declaration of measles outbreak in the City;

WHEREAS, the declaration  is indispensable to facilitate holistic and swift responsive actions to protect the health, welfare and well-being of the people of Olongapo as well our neighbor communities;
                         
NOW, THEREFORE, on motion of  City Councilor Eduardo Guerrero, with the unanimous accord of the Members of the Sanggunian Panlungsod  present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the  Sangguniang Panlungsod in session assembled to declare, as it hereby declares a Measles Outbreak in Olongapo City.

            Let a copy of the City Health status report be attached and  made as an integral part of this Resolution.

A RESOLUTION ADOPTING REPUBLIC ACT NO. 10606 “AN ACT AMENDING R.A 7875 OTHERWISE KNOWN AS THE “NATIONAL HEALTH INSURANCE ACT OF 1995”, AS AMENDED, AND FOR OTHER PURPOSES” AND ENJOINING ALL CONCERNED ESTABLISHMENTS AND OFFICES ON ITS IMPLEMENTATION

            WHEREAS, Section 15, Article II of the 1987 Philippine Constitution declares that the State shall protect and promote the right to health of the people and instill health consciousness among them.

WHEREAS, Section 11, Article XIII of the 1987 Philippine Constitution declares that the state shall adopt an integrated and comprehensive approach to health development which shall endeavor to make essential goods, health and other services available to all people at affordable cost.  Priority for the need of the underprivileged sick, elderly, disabled, women and children shall be recognized.  Likewise, it shall be the policy of the state to provide free medical care to paupers.

WHEREAS, Republic Act 7875, also known as the National Health Insurance Act of the Philippines instituting a National Health Insurance Program for all Filipinos and establishing the Philippine Health Insurance Corporation for the purpose.

WHEREAS, Republic Act 10606, which is a consolidation of Senate Bill 2849 and House Bill 6048, was finally passed by the Senate and House of Representatives on February 4, 2013.

WHEREAS, the law mandates the State to provide comprehensive health care services to all Filipinos through a socialized health insurance program that will prioritize the health care needs of the underprivileged, sick, elderly, persons with disabilities (PWDs), women and children and provide free health care services to indigents.

WHEREAS, the new law provides that all citizens of the Philippines shall be covered by the National Health Insurance Program (NHIP). The program shall ensure sustainability of coverage and continuous enhancement of the quality of services to people.  The program shall be compulsory in all provinces, cities and municipalities nationwide, notwithstanding the existence of local government unit-based health insurance programs.

WHEREAS, the Olongapo City legislative body as it recognizes the beneficent features of RA 7875 to greatly improve the health status and quality of life of each sector the community , further strengthen its support to adopt the National Health Insurance Program (NHIP) implementation;

NOW, THEREFORE, on motion of City Councilor Benjamin G. Cajudo II, co-sponsored by City Councilor Eduardo G. Guerrero, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in the session assembled, to adopt, as it hereby adopts Republic Act Number 10606 An Act Amending Republic Act 7875 Otherwise Known as the National Health Insurance Act of 1995 as amended and for other purposes and enjoining all concerned establishments and offices on its implementation

A RESOLUTION FERVENTLY COMMENDING THE PHILIPPINE RED CROSS (PRC) THRU CHAIRMAN RICHARD J. GORDON FOR THE IMMEASURABLE SERVICE & HUMANITARIAN ASSISTANCE IT PROVIDED TO THE PEOPLE OF OLONGAPO DURING THE ONSLAUGHT OF TYPHOON ODETTE, THE UNPRECEDENTED HABAGAT FLOOD AND THE RESULTING LEPTOSPIROSIS OUTBREAK


WHEREAS, Olongapo City was submerged by almost biblical and unprecedented floods brought about by Typhoon Odette & heavy rains of the Habagat season in the unlikely hours of September 23, 2013;

WHEREAS, the extraordinary flood waters caused damages to both lives & property, resulting in death, inundating homes & vehicles and leaving most houses and streets in knee deep mud & stagnant waters;

WHEREAS, the Philippine Red Cross thru Chairman Richard J. Gordon, sprung into action and assisted in the rescue, relief & recovery efforts by deploying PRC rescue volunteers to search for victims and heavy equipments to clear the affected barangays of dirt & debris;

WHEREAS, the Philippine Red Cross team worked side by side with the City Government of Olongapo in extending relief & recovery efforts to the victims of this city-wide calamity. Along with the cooperation & donations from kind-hearted individuals, socio-civic and non-governmental organizations, the resurgence of Olongapo was ensured when government & the people worked hand in hand;

WHEREAS, another ensuing crisis developed when people became exposed to disease borne flood waters & bacteria-laden dirt caused more than 600 patients to be stricken with leptospirosis, unfortunately resulting to the death of 13 individuals;

WHEREAS, the Philippine Red Cross continued its assistance to Olongapo by opening its blood bank, providing cot beds, sending PRC Volunteer nurses and setting up tents with medical equipment  to help treat the leptospirosis victims that almost overwhelmed the facilities of the city run hospital;

WHEREAS, the provided equipment & manpower of the Philippine Red Cross aided in the clean-up of  streets & schools of Olongapo, thereby restoring normalcy to the lives of the affected residents;

WHEREAS, the timely humanitarian assistance provided by the Philippine Red Cross helped fast track the recovery of the city, alleviating the hardships of the people brought on by this historic flood;

WHEREAS, the Philippine Red Cross thru Chairman Richard J. Gordon, the son of Olongapo, true to its mandate, provides humanitarian action in times of national crisis, disasters, emergencies & conflicts without fear or favor;

NOW, THEREFORE, on motion of City Councilor Aquilino “Jong” Y. Cortez, Jr. with the unanimous accord of the Members of the Sangguniang Panglungsod present, namely: City Councilor Eyrma Yvette  Marzan-Estrella, City Councilor Eduardo G. Guerrero, City Councilor Benjamin G. Cajudo II, City Councilor Elena C. Dabu, City Councilor Noel Y. Atienza, City Councilor Alreuela M. Bundang-Ortiz, City Councilor Edna A. Elane, City Councilor Emerito Linus D. Bacay, City Councilor Carlito A. Baloy and City Councilor Dolly Mae M. Ramos.

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to commends, as it hereby commends fervently the Philippine Red Cross (PRC) thru Chairman Richard J. Gordon for the immeasurable service and humanitarian assistance it provided to the people of Olongapo during the onslaught of Typhoon Odette, the unprecedented Habagat Flood and  the resulting leptospirosis outbreak.

A RESOLUTION EXPRESSING GRATITUDE TO HONORABLE JEJOMAR C. BINAY,  VICE PRESIDENT OF THE REPUBLIC OF THE PHILIPPINES, FOR DONATING DOXYCYCLINE, IV FLUIDS AND RELIEF PACKAGES TO OLONGAPO CITY

WHEREAS, leptospirosis, a deadly bacterial infection, is commonly transmitted to human from contaminated water caused usually by rat urine, and symptoms appear only several days up to two (2) weeks post exposure to contaminated floodwater;

WHEREAS, an outbreak of leptospirosis in the City of Olongapo happened two weeks after the heavy flooding in the city brought about by the southwest monsoon rains on September 23, 2013;

WHEREAS, on October 10, 2013 at 2:30 in the afternoon, the Honorable Members of the City Council, in their 3rd Special Session, passed a resolution declaring the leptospirosis outbreak in Olongapo City. It was reported that the city reached its peak of 631 cases of leptospirosis, and treatment to admitted patients are still on-going at the James L. Gordon Memorial Hospital (JLGMH);
                                                                                                  
WHEREAS, Vice-President Jejomar C. Binay donated to James L. Gordon Memorial Hospital 24 boxes of Doxycycline, a tetracycline antibiotic that fights bacteria in the body, and 30 boxes of IV fluids, used to prevent dehydration in patients. This initiative from the Vice-President Binay is timely due to shortage in supply of the medications needed by patients with leptospirosis;

WHEREAS, in addition to the medications, Vice-President Binay likewise donated 2,000 packages of relief goods for flood victims;

WHEREAS, the City of Olongapo conveys profound appreciation to the act of kindness extended by Vice-President Binay;

NOW, THEREFORE,  with the unanimous accord of the Members of the Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to express, as it hereby expresses gratitude to Honorable Jejomar C. Binay, Vice-President of the Republic of the Philippines, for donating Doxycycline and IV Fluids and relief packages to Olongapo City.

WHEREAS, the United Coconut Planters Bank Life Assurance Corporation, is a company engaged in the promotion and marketing of its health care programs consisting of preventive health care, in-patient and out-patient services;

WHEREAS, the James L. Gordon Memorial Hospital is a tertiary medical facility owned by the Local Government of Olongapo City;

WHEREAS, the aforesaid company can only accomplish its tasks through its accreditation to a hospital that provides excellent medical facility and quality care, such as the James L. Gordon Memorial Hospital;

WHEREAS, the City deems to authorize the City Mayor for and on behalf of the James L. Gordon Hospital to enter into an agreement with the United Coconut Planters Bank Life Assurance Corporation regarding their health care package;

NOW, THEREFORE, on motion of City Councilor Eduardo G. Guerrero, with the unanimous accord of the members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to authorize, as it hereby authorizes the City Mayor, Honorable  Rolen C. Paulino, to enter into a mutual accreditation agreement for and on behalf of the James L. A RESOLUTION AUTHORIZING THE CITY MAYOR HONORABLE ROLEN C. PAULINO TO  ENTER INTO A MEMORANDUM AGREEMENT (MOA) FOR AND ON BEHALF OF  JAMES L. GORDON MEMORIAL HOSPITAL (JLGMH) WITH STA. CECILIA  MEDICAL  CENTER   (SCMC)  REPRESENTED  BY  ITS  HOSPITAL ADMINISTRATOR, DR. ADONIS M. CALAYCAY, DESIGNATING JLGMH
 AS SCMC’S  REFERRAL HOSPITAL

            WHEREAS,  the Department of Health mandated Sta. Cecilia Medical Center of Iba, Zambales to enter into agreement with a tertiary hospital as its referral hospital;

WHEREAS,  James L. Gordon Memorial Hospital (JLGMH) is one of the most equipped tertiary training hospital in the Olongapo City-Zambales area in terms of medical services and facilities, and has become among the most in-demand medical facilities in the Central Luzon area;

WHEREAS, Sta. Cecilia Medical Center has previously entered into an agreement with the James L. Gordon Memorial Hospital through the Olongapo City Government designating JLGMH as its referral hospital;

WHEREAS, Sta. Cecilia Medical Center intend to renew its agreement with the City Government of Olongapo to designate JLGMH as its referral hospital and has thus requested JLGMH to grants its request in formalizing its existing referral system with JLGMH in their common thrust for providing optimum health care;
  
NOW, THEREFORE, on motion of City Councilor  Eduardo G. Guerrero, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to authorize, as it hereby authorizes the City Mayor Honorable Rolen C. Paulino to  enter into a Memorandum of Agreement (MOA) for and on behalf of  James L. Gordon Memorial Hospital (JLGMH) with Sta. Cecilia Medical Center  (SCMC) represented by its Hospital Administrator, Dr.Adonis M. Calaycay, for the renewal of designating JLGMH as SCMC’s referral hospital.

A RESOLUTION AUTHORIZING THE CITY MAYOR  HONORABLE ROLEN C. PAULINO TO ENTER INTO AND SIGN THE MEMORANDUM OF AGREEMENT FOR AND ON BEHALF OF THE JAMES L. GORDON MEMORIAL HOSPITAL WITH PNEUMOCARE LUNG PHILS, INC. (PLPI), RELATIVE TO THE TIE UP FOR THE OPERATION OF THE PULMONARY DIVISION OF THE JLGMH

            WHEREAS,     the members of the Special Prequalification, Bids and Awards Committee  unanimously recommended that it is advantageous for the City to enter into a Memorandum of Agreement with PNEUMOCARE LUNG PHILS,INC (PLPI), following the  attached results of the Evaluation of Proposal to Operate the JLGMH Pulmonary Division;

            WHEREAS,  with the assessment, it is deemed that an agreement be forged for the swift implementation of the same;

            WHEREAS, Pneumocare Lung Phils., Inc. (PLPI) further commits itself to corporate social responsibility by extending assistance to the indigent patients referred by the Office of the Vice Mayor and Members of the City Council through a letter of indorsement, in addition to the stipulated accommodation to the Letters of Guarantee issued by the Office of the City Mayor as contained in the commitment and obligations of PLPI in the Memorandum of Agreement;

NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present; 
RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the  City Mayor Honorable  Rolen C. Paulino to and enter into and sign the Memorandum of Agreement for and on behalf of the James L. Gordon Memorial Hospital (JLGMH) with Pneumocare Lung Phils., Inc. (PLPI), relative to the tie up for  the operation of the Pulmonary Division of the James L. Gordon Memorial Hospital.

A RESOLUTION AUTHORIZING THE CITY MAYOR, HON. ROLEN C. PAULINO TO SIGN AND ENTER INTO A CONTRACT OF AFFILIATION, FOR AND ON BEHALF OF THE GORDON COLLEGE, THIS CITY  WITH SAN LAZARO HOSPITAL REPRESENTED BY DR. WINSTON S. GO RELATIVE TO THE TRAINING
OF NURSING STUDENTS

WHEREAS, the Gordon College is a tertiary level learning institution, duly authorized and recognized by the Commission on Higher Education to accept enrollees for Health Science and related Programs;

WHEREAS, the pressing demand for health professionals in the global market  led to the growing number of enrollees to the nursing course and a tertiary-care capable hospital is necessary in complying to the Nursing Act of 2002 that requires Nursing students to undergo Related-Learning- Experience (duty hours) in the community as well as in hospital setting;

WHEREAS, the San Lazaro Hospital is an authorized capable hospital that has the capability and the facilities to impart the real hospital set up and exposures required of a Nursing job to the students;

WHEREAS, San Lazaro Hospital agrees and allows the affiliation and training of Nursing students of  Gordon College subject to the policies, procedures and guidelines governing Affiliation and Training of Students as prescribed by the Department of Health/ Hospital/Rural Health Units and Other Health Agencies;
  
WHEREAS, Gordon College shall provide the grading system and the grading sheets to be used by San Lazaro Hospital for the Related-Learning-Experience (RLE) exposure;

NOW, THEREFORE, on motion of  City Councilor Eduardo G. Guerrero, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes City Mayor, Hon. Rolen C. Paulino to sign and enter into a Contract of Affiliation, for and on behalf of the Gordon College, this City  with San Lazaro Hospital represented by Dr. Winston S. Go, relative to the training of nursing students.

A RESOLUTION  DECLARING THE OUTBREAK
OF LEPTOSPIROSIS IN OLONGAPO CITY

WHEREAS, Olongapo City was submerged in flood water due to the heavy rains brought about by Typhoon Odette and Habagat;

WHEREAS, the unexpected heavy rainfall of Typhoon Odette and Habagat not only destroyed properties but has also caused immense health danger;

WHEREAS, there is a sudden increase in Leptospirosis cases reported at City Health Office resulting from the victims’ exposure to flood water and soil infected with Leptospira Bacteria found on animal urine;

WHEREAS, as per Committee Report of Councilor Eduardo G. Guerrero, Chairman of Committee On Health, there are One Hundred Eighty Two (182) cases of Leptospirosis including Five (5) death casualties at James L. Gordon Memorial Hospital, Four (4) residents from Olongapo City and One (1) resident from Subic, Zambales, as of 6:00 A.M. of October 09, 2013;

WHEREAS, as of date, October 10, 2013, there is a recorded Three Hundred Twelve (312) Leptospirosis patients at James L. Gordon Memorial Hospital with Seven (7) death casualties, Six (6) of which are Olongapo residents as per Dr. Rodrick Bustamante, Officer in Charge of the City Health, said rapid increase in number unanticipated by the City’s Health Authorities despite massive city-wide prophylactic measure conducted daily;

NOW, THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to declare, as it hereby declares the Outbreak of Leptospirosis in Olongapo City.
