A RESOLUTION AUTHORIZING THE CITY GOVERNMENT OF OLONGAPO REPRESENTED BY CITY MAYOR JAMES GORDON, JR. TO ENTER INTO A MEMORANDUM OF AGREEMENT WITH JIMEC GLOBAL HOLDINGS,
INC. REPRESENTED BY ITS PRESIDENT MS. ARABELLA F. ULAYE REGARDING THE (PTC2S-PUBLIC TRANSPORT COLOR CODED SCHEME, PATS-PUBLIC ASSISTANCE TRACKING SYSTEM,VMS-VOLUNTEER MANAGEMENT SYSTEM)

WHEREAS, the City of Olongapo pioneered in the implementation of the first color coded transportation system and became the model to all local government units in the Philippines;

WHEREAS, the city recognizes the advantages of harnessing Information and Communications Technology as a tool to enhance the delivery of basic services;

WHEREAS, JIMEC Global Holdings, Inc. is an ICT corporation with an extensive experience in providing ICT solutions which are designed to facilitate an innovative and effective way of managing the city’s public transport vehicles, drivers and operators, public assistance services and volunteer services;

NOW, THEREFORE, on motion of Councilor Cynthia G. Cajudo, with the unanimous accord of the Members of the Sangguniang Panlungsod present,


RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the City Government of Olongapo represented by City Mayor James Gordon, Jr. to enter into a Memorandum of Agreement with JIMEC Global Holdings, Inc. represented by its President Ms. Arabella F. Ulaye regarding the (PTC2S-Public Transport Color Coded Scheme, PATS-Public Assistance Tracking System, VMS-Volunteer Management System).

AND THE COLUMBAN COLLEGE – COLLEGE OF NURSING & SCHOOL OF MIDWIFERY REPRESENTED BY ITS ACTING PRESIDENT REV. MSGR. CRISOSTOMO CACHO, VGPC, REGARDING HEALTH PROGRAMS


WHEREAS, the Olongapo City Health Department and Barangay Health Centers, with their provision of health services, directly benefit the general public;

WHEREAS, the affiliation of nursing and midwifery students from Columban College to the City Health Department will not only help train students but will also bring additional healthcare providers to serve the public;

NOW, THEREFORE, with the unanimous accord of the Members of Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to ratify, as it hereby ratifies the Memorandum of Agreement Entered into by and Between the City Government of Olongapo Represented by City Mayor James Gordon, Jr. and the Columban College – College of Nursing & School of Midwifery Represented by its Acting President Rev. WHEREAS, Fortune Medicare, Inc. requested to enter into an Affiliation Agreement with the James L Gordon Memorial Hospital in order to provide the benefits and services of its healthcare program to the enrolled members and dependents, through the use of the City Hospital’s facilities and services;

NOW, THEREFORE, on motion of Councilor John Carlos G. delos Reyes, with the unanimous accord of the Members of Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to authorize, as it hereby authorizes the City Government of Olongapo represented by Mayor James Gordon, Jr. to enter into a Memorandum of Agreement with the Fortune Medicare, Inc. represented by its Executive Vice President and General Manager Dorothea J. WHEREAS, the aforementioned summer activity under the Olongapo Business Club, Inc. is one of the effective promotional activities for the City tourism industry;

WHEREAS, the Sangguniang Panlungsod Members deem it proper to ratify the Memorandum of Agreement (MOA);

NOW, THEREFORE, on motion of Councilor Gina G. Perez, with unanimous accord of the Members of Sangguniang Panlungsod present;

A RESOLUTION RATIFYING THE MEMORANDUM OF AGREEMENT ENTERED INTO BY AND BETWEEN DIVINE SPIRIT HOSPITAL AND COLLEGES, INC. REPRESENTED BY DR. ARTURO N. IMBUIDO AND THE CITY HEALTH DEPARTMENT REPRESENTED BY MAYOR JAMES GORDON, JR.


WHEREAS, part of the Olongapo City Government’s commitment to provide optimum public health and welfare, and safety of its citizens is the support it gives to health care providers;

WHEREAS, the affiliation and training of nursing students and other health related courses to our City Health Department provides additional health care providers to our health centers and gives educational support to the students studying in the city to earn significant learning experience;

WHEREAS, a resolution authorizing the Memorandum of Agreement entered into by the City Health Department of Olongapo represented by Hon. James Gordon, City Mayor and Divine Spirit Hospital and Colleges, Inc. represented by Dr. Arturo N. Imbuido, School President was signed regarding their affiliation and training of the practical nursing and caregiver students of the said school;

NOW, THEREFORE, on motion of Councilor Marey Beth D. Marzan and co-sponsored by Councilor Angelina B. Andrada with the unanimous accord of Members of the Sangguniang Panlungsod;

RESOLVED, AS IT HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled, to ratify, as it is hereby ratifies the Memorandum of Agreement entered into by and between Divine Spirit Hospital and Colleges, Inc. represented by Dr. Arturo N. Imbuido and the City Health Department represented by Mayor James Gordon, Jr.

A RESOLUTION AUTHORIZING THE CITY MAYOR TO ENTER INTO A MEMORANDUM OF AGREEMENT (MOA) FOR THE AFFILIATION OF ST. JOSEPH COLLEGE WITH JAMES L. GORDON MEMORIAL HOSPITAL

WHEREAS, the James L. Gordon Memorial Hospital (JLGMH), a tertiary training hospital owned and managed by the City Government of Olongapo, endeavors to provide excellent medical and health care services to the people of Olongapo as well as its neighboring towns and provinces;

WHEREAS, awarded by the Department of Health (DOH), as a Sentrong Sigla Hospital, the JLGMH now stands as a training ground where students enrolled in nursing and in other allied medical courses can be trained in various specialty areas at JLGMH, to comply with the requirements of the law for them to undergo Related-Learning Experience (duty hours);

WHEREAS, St. Joseph College – Olongapo City is offering Master of Arts in Nursing (MAN) in consortium with the Bataan Polytechnic State College (BPSC). Students enrolled therein are likewise required by law to undergo actual training in administrative level in different specialty areas in hospitals;

WHEREAS, the said college requests that their students of MAN be admitted and allowed to undergo actual training at the James L. Gordon Memorial Hospital;

WHEREAS, the City approves the aforesaid request of the St. Joseph College provided that the training intended for the students of the Gordon College, which is owned and operated by the City, will not be jeopardized;

WHEREAS, Sangguniang Panlungsod deems it beneficial to authorize the City Mayor to enter into the said affiliation agreement to pave way for the said training;

WHEREAS, the Columban College, is an institution duly recognized by the Commission on Higher Education, intends to affiliate their interns and students from the College of Nursing and School of Midwifery to the City’s Barangay Health Centers for their Related Learning Experience;

WHEREAS, the students, as adjunct to the health care providers may be able to help in community health services and in the implementation of different Department of Health (DOH) programs;

WHEREAS, Columban College administration for their part, must ensure close supervision of their students, render genuine and compassionate health care service, and abide with the policies and guidelines of the City Health Department;
