WHEREAS, the City Mayor has appointed Atty. Anna Marin F. Sison as City Government Department Head II - City Legal Officer (Co-terminus) pursuant to Civil Service Laws, Rules, and Regulation;

WHEREAS, Section 454 (d) of RA 7160, otherwise known as the Local Government Code of 1991, provides that heads of departments shall be appointed by the City Mayor with the concurrence of the majority of all members of the Sangguniang Panlungsod.

A RESOLUTION CONFIRMING THE APPOINTMENT OF MR. WILLY R. ANTIGO AS A MEMBER OF THE GORDON COLLEGE-BOARD OF TRUSTEES

WHEREAS, pursuant to city Ordinance No. 9, Series of 1999, Olongapo City Colleges, now known as Gordon College, was created with the mandate of providing excellent quality education for the poor and deserving students of Olongapo City;

WHEREAS, this responsibility is carried out mainly by the school’s Board of Trustees, comprised by qualified persons with academic and technical expertise in formulating policies and decisions for the attainment of the institutions’ mission and vision ;

WHEREAS, pursuant to Section 4 of City Ordinance No. 8, Series of 2010, the Board of Trustees shall be composed of the President of the Faculty Association;

WHEREAS, Mr. Willy R. Antigo has been elected as the President of the Gordon College Faculty Association for the academic year 2012-2013 succeeding Mr. Leby A. Fastidio, making him the official representative of the college’s teaching staff to the Board of Trustees;

WHEREAS, Mr. Antigo, an instructor under the college of Arts and Sciences, has been serving the college for over a decade, five years of which were spent as the Dean of the said department. Moreover, Mr. Antigo also advised college organizations and handled the vice presidency of the Faculty Association for two terms (2001-2002, 2011-2012);

WHEREAS, in the exigencies of service, Mr. Antigo was appointed as an interim member of the Board of Trustees pending compliance with pertinent legislative requirements;

WHEREAS, by reason of his experience and long-term service as a faculty member of the college, he embodies the values and understanding worth emulating by both co-educators and students of the college, and thus was formally nominated for membership on the said board subject to the required confirmation under pertinent laws;

WHEREAS, pursuant to City Ordinance No. 9, Series of 1999, Olongapo City Colleges now known as Gordon College, was created with the mandate of providing excellent quality education for the poor and deserving students of Olongapo City;

WHEREAS, this responsibility is carried out mainly by the college’s Board of Trustees, comprised by qualified persons including the president of the duly recognized Student Council as specified in Ordinance No. 8, Series of 2010, who will formulate policies and decisions for the attainment of the institution’s mission and vision ;

WHEREAS, Mr. Kiel Nixon Montano currently serves as the Chairman of the Gordon College-Supreme Student Council (GC-SSC), making him a nominee to represent the student body to the Board of Trustees;

WHEREAS, MR. Montano, a Hotel and Restaurant Management student of the College also served as the Treasurer of the GC-SSC before assuming office as the Chairman, where he demonstrated leadership emulated by the studentry;

WHEREAS, in the exigencies of service, Mr. Montano was appointed as interim member of the Board of Trustees pending compliance with pertinent legislative requirements;

A RESOLUTION CONFIRMING THE BIDDING RESULTS AND ISSUANCE OF A NOTICE

OF AWARD TO THE BEACON MARKETING FOR THE DEVELOPMENT OF THE SLAUGHTERHOUSE AND AUTHORIZING THE HONORABLE CITY MAYOR JAMES GORDON, JR. TO ENTER, EXECUTE AND SIGN THE MEMORANDUM OF AGREEMENT WITH BEACON MARKETING, IN A MANNER CONSISTENT WITH LAW

WHEREAS, on May 17, 2012 Beacon Marketing as represented by the General Manager, Mr. Patrick Philip A. Pineda submitted an unsolicited proposal essentially moving for the upgrading and development of the Olongapo City Slaughterhouse under the following terms and conditions: (a) Offered the payment of an Annual Franchise fee of Php500,000.00 or 15% of the gross slaughter fees, whichever is higher; (b) Committed investment and improvement amounting to Php20,600,000.00; (c) Shouldering all cost to be uncured in the construction, operation, repairs and maintenance, and management of the slaughterhouse for the contract period; (d) Contract Duration for 25 years; (e) Payment to be remitted to the city through the City Treasurer’s Office on a quarterly basis, and which shall start on the first of the operation of the slaughterhouse computed from the first day of operation by the proponent; (f) The ante mortem and post mortem fees shall go directly to the city Government through the City Veterinary Office, the rate of which will be determined by the city; (g) The proponent will impose increase in the necessary fees or charges in the following schedules upon operation of the new and renovated slaughterhouse, provided they already got the “AA” accreditation of the NMIS; (h) Retention of 1^st and 2^nd year fees (i.e. fees will be retained0Hogs: Php69 per head plus Php23 for ante and post mortem inspection fees; Large Animals: Php185 per head plus Php40 for ante and post mortem inspection fees- 3^rd to 5^th year: Fees will be adjusted (Hogs: Php97 per head plus Php23 for ante and post mortem inspection fees; Large Animals: Php210 per head plus Php40 ante and post mortem inspection fees); 6^th to 10^th year: Fees will be adjusted to Php108.64 per head of hog and Php235.30 per head for large animals plus adjusted ante and post mortem inspection fees must be adjusted upon amending the City Ordinance; 16^th and 20^th year: Fees will be adjusted to Php136.28 per head of hogs and Php295.03 per head of large animals plus adjusted ante and post mortem inspection fees must be adjusted upon amending the City Ordinance: 21^st to 25^th year: Fees will be adjusted to Php152.63 per head of hogs and Php330.44 per head of large animals plus adjusted ante and post mortem inspection fees must be adjusted upon amending the City Ordinance.

WHEREAS, the Sta. Cecilia Medical Center (SCMC) has an affiliation agreement with the City of Olongapo which has already expired and must be renewed to ensure the continuation of cooperative relationship and referral system with the James L. Gordon Memorial Hospital to improve patient services and achieve quality patient outcomes based on equity, efficiency and cost-effectiveness with a view of improving exchange of patient clinical information flow between the two medical facilities;

NOW, THEREFORE, on motion of City Councilor Edna A. Elane, with the unanimous accord of the Members of the Sangguniang Panlungsod present;

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to authorize, as it is hereby authorizes, the Honorable City Mayor James Gordon, Jr. to Enter, Execute and Sign a Memorandum of Agreement for the Renewal of the Affiliation Agreement with the Sta. WHEREAS, this responsibility is carried out mainly by the school’s Board of Trustees, comprised by qualified persons with academic and technical expertise in formulating policies and decisions for the attainment of the institution’s mission and vision;

WHEREAS, Dr. Akiate currently serves as Director IV of the Commission on Higher Education Regional Office III (CHEDROIII), covering the 185 colleges and universities in Pampanga, Bulacan, Tarlac, Bataan, Nueva Ecija, Aurora and Zambales;

WHEREAS, Dr. Akiate also served as Chief Education Program Specialist as CHED-Cordillera Administrative Region (CAR) as well as OIC-College President at Surigao State College of Technology, and have significantly contributed in the operations of both institutions;

WHEREAS, in the exigencies of service, Dr. Akiate was appointed as interim member of the Board of Trustees pending compliance with pertinent legislative requirements;

A RESOLUTION CONFIRMING THE APPOINTMENTS OF CITY COUNCILORS EYRMA YVETTE MARZAN-ESTRELLA, EDUARDO J. PIANO, AND RODEL S. CEREZO AS MEMBERS THE GORDON COLLEGE – BOARD OF TRUSTEES

WHEREAS, pursuant to City Ordinance No. 9, Series of 1999, Olongapo City Colleges, now known as Gordon College, was created with the mandate of providing excellent quality education for the poor and deserving students of Olongapo City;

WHEREAS, this responsibility is carried out mainly by the school’s Board of Trustees, comprised by qualified persons with academic and technical expertise in formulating policies and decisions for the attainment of the institution’s mission and vision;

WHEREAS, pursuant to Section 4 of City Ordinance No. 8, Series of 2010, the Board of Trustees shall be composed of the Chairmen on Education and Labor and Employment, and prominent citizens who have distinguished themselves in education ;

WHEREAS, City Councilor Eyrma Yvette Marzan – Estrella and Eduardo J. Piano hold the chairmanships of Education and Labor and Employment respectively, while Councilor Rodel S. Cerezo is an active educator and a member of the Education Committee;

WHEREAS, in the exigencies of service, City Councilors Eyrma Yvette Marzan-Estrella, Eduardo J. Piano and Rodel S. Cerezo were appointed as interim members of the Board of Trustees pending compliance with pertinent legislative requirements;

WHEREAS, by reason of their vast experience as legislators, they epitomize the values which are worth emulating and were formally nominated for membership on the said board subject to the required confirmation under pertinent laws;

NOW, THEREFORE, on motion of City Councilor Elena C. Dabu, with the majority decision of the Members of the Sangguniang Panlungsod present;

WHEREAS, the City Mayor has appointed Mr. Fernando M. Magrata as City Government Department Head II (City Administrator) under the City Administrator’s Office pursuant to Civil Services Laws, Rules and Regulations;

WHEREAS, Section 454(d) of RA 7160, otherwise known as the Local Government Code of 1991, provides that heads of departments shall be appointed by the City Mayor with concurrence of the majority of all Members of Sangguniang Panlungsod.

NOW THEREFORE, with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLUTION NO. 131
(Series of 2007)

A RESOLUTION CONFIRMING AND RATIFYING THE PLACEMENT OF PABX SYSTEM IN THE OLD BUILDING AND THE REST OF THE 20 LINES IN THE NEW BUILDING AS RECOMMENDED BY THE CHIEF OF HOSPITAL, DR. ARTURO MENDOZA


WHEREAS, by virtue of Resolution No. 36, Series of 2005, an authority was conferred to the City Mayor Hon. James Gordon, Jr. to enter into a Contract of Loan with the Development Bank of the Philippines that will finance the upgrading of equipment and facilities of James L. Gordon Hospital;

WHEREAS, the said resolution was ratified by the same Council through the adoption of Resolution No. 55, Series of 2006;

WHEREAS, Dr. Arturo Mendoza as the Chief of Hospital decided to partially place the PABX system in the old building and the rest of the lines in the new Medical Arts Building. He justified that the old building needs more lines than the said new building and that the old PABX system shall still be used also in the new building;

WHEREAS, both contracting parties had agreed to the changes and can be deduced by the fact that DBP Board has approved to the installation of the new PABX system in the main building;

WHEREAS, the letter of Dr. Arturo Mendoza addressed to Ms. Myrna Reinoso of DBP last February 17, 2007 and Dr. Arturo Mendoza’s request letter to the Sangguniang Panlungsod shall be made an integral part of this resolution;

NOW, THEREFORE, on joint motion of Councilors Marey Beth D. Marzan and Angelito W. Baloy with the unanimous accord of the Members of the Sangguniang Panlungsod present,

RESOLVED, AS IT IS HEREBY RESOLVED, by the Sangguniang Panlungsod in session assembled to confirm and ratify, as it hereby confirms and ratifies the placement of the PABX system in the old building and the rest of the 20 lines in the new building as recommended by the Chief of Hospital, Dr. Arturo Mendoza.
